We are in-network with Aetna commercial plans. To confirm eligibility, please contact your insurance company and upload a copy of your insurance card when booking your appointment.

Cigna

United Healthcare

AHCCCS & Financial Hardship

Currently, AHCCCS does not provide lactation coverage. If you have AHCCCS or are uninsured and experiencing financial hardship, Lavender & Latch offers a sliding scale. For more information on the sliding scale, contact Erica at erica@lavenderandlatch.com or (602) 691-5862.

Other Insurance Plans

We are currently in the process of getting in-network with Blue Cross Blue Shield AZ, TriCare, and Humana. If you have one of these plans or a plan not listed above, you may still be able to receive reimbursement by submitting a superbill to your insurance provider after your visit.
